Cancellation Policy

I agree to keep all scheduled appointments and be on time. If I cannot attend a scheduled session, I will contact Document > Practice Name to cancel and/or reschedule as soon as possible. There will be no fee if the appointment is rescheduled for the same week or is canceled 24 hours before the scheduled appointment time. I understand if I miss an appointment or cancel with less than 24 hours of notice, I will be charged a cancellation fee of $50.00 to the credit card on file unless I am covered by a Washington Apple Health plan. In that case, more than 2 missed appointments may limit your ability to schedule in the future. A missed appointment includes arriving late without provider approval, technical issues preventing audio and video connection or failing to attend the appointment without communication. If these conditions are not met, we are unable to complete a nutritional assessment and submit a claim to insurance. Because "life happens" and missing an appointment isn't always within your control, every client will be allowed one late cancel/missed appointment per 6 months without a charge. Late fees are charged at the provider's discretion.

Financial Policy

I understand that I am obligated to ensure that all fees are paid in full.  Michelle Weinbender Nutrition PLLC will verify my coverage and bill my insurance carrier on my behalf. However, I am ultimately responsible for the payment of my bill. It is my responsibility to know and verify my insurance coverage for Medical Nutrition Therapy. ​
