Cancellation Policy

I agree to keep all scheduled appointments and be on time. If I cannot attend a scheduled session, I will contact Michelle Weinbender Nutrition PLLC to cancel and/or reschedule. There will be no fee if the appointment is rescheduled for the same week or is canceled 24 hours before the scheduled appointment time. I understand if I miss an appointment or cancel with less than 24 hours of notice, I will be charged a cancellation fee of $50.00 to the credit card on file unless I am covered by a Washington Apple Health plan. In that case, missed appointments may limit your ability to schedule in the future. Because "life happens" and missing an appointment isn't always within your control, every client will be allowed one late cancel/missed appointment per 12 months without a charge.

Financial Policy

I understand that I am obligated to ensure that all fees are paid in full.  Michelle Weinbender Nutrition PLLC will verify my coverage and bill my insurance carrier on my behalf. However, I am ultimately responsible for the payment of my bill. It is my responsibility to know and verify my insurance coverage for Medical Nutrition Therapy. ​
